Job description

Position will be responsible for providing initial and annual there after assessments of Individuals residing in Residential Waiver Group homes. Position will develop and ensure implementation and updating of Individual Support Plans assigned as needs change. Successful candidate will track and monitor implementation of individual support plans for individuals with intellectual by effective communication skills with those implementing plan. Candidate will be responsible for training staff on implementation of all developed individual support plans. Candidate will be expected to travel via catchment area assigned - Driver's License and safe driving record required. Candidate must have efficient computer skills and working knowledge of Human Services. Successful candidate must have a Bachelor's degree in Human Services field from an accredited college and at least one year of paid experience working with individuals who have intellectual disabilities. Full Time with Benefits Package.
Must be willing to consent to background checks, including checks for barrier crimes, and drug screening as part of the application process.
