For nearly two decades, Consumer Direct Care Network Arizona (CDAZ) has been a trusted provider of home care services to Arizonans eligible for the Arizona Medicaid Personal Care Services Program. We specialize in offering personalized caregiving that supports individuals in maintaining their independence and quality of life within the comfort of their own homes. Through our dedicated services, we empower individuals to remain engaged in their communities while receiving the care they deserve.

Responsibilities
  • Assist individuals with activities of daily living in private homes
  • Help with personal care, meal preparation, and light housekeeping
