Key Responsibilities

• Conduct comprehensive assessments to identify clients’ social, emotional, and environmental challenges, utilizing telehealth tools to gather necessary information effectively.
• Develop and implement individualized service plans that include specific goals, interventions, and methods for monitoring progress.
• Provide compassionate counseling to individuals and families grappling with various life stressors, employing evidence-based practices tailored to client needs.
• Foster collaborative relationships with clients to create personalized service plans and facilitate access to critical community resources.
• Engage in crisis intervention efforts, performing risk assessments and developing safety plans as required to ensure client well-being during challenging times.
• Work closely with community agencies to coordinate comprehensive services, advocating on behalf of clients to maximize available resources.
• Participate in outreach initiatives to raise awareness about our agency’s services and promote community engagement.
• Take part in team meetings and training sessions, contributing to a culture of shared knowledge and continuous improvement.

Patient Population / Client Focus

You will provide support primarily to individuals and families facing diverse challenges, including but not limited to mental health concerns, socioeconomic hardships, and domestic issues. By understanding the unique backgrounds and experiences of our clients, you will be able to craft targeted intervention strategies that address their needs holistically. Your role will directly impact their quality of life and resilience in the face of adversity.

Qualifications

• A Bachelor's Degree in Social Work from an accredited program is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of social, economic justice issues, trauma-informed care, and available community resources.
• Exceptional communication skills, including cultural competence and problem-solving abilities to effectively interact with diverse populations.

Preferred Experience

• At least 2 years of experience working in a social service agency or mental health counseling setting, preferably leveraging telehealth platforms.
• Familiarity with case management software and understanding of evidence-based practices in counseling.
• Previous involvement in community outreach programs and advocacy efforts is a plus.

Compensation and Benefits
