Work Environment

This position is remote, providing flexibility in your work setup while allowing you to connect directly with clients who need your support. You will mainly interact with clients in the hospice center environment, aiming to create a nurturing and trusting atmosphere where they feel safe to express their thoughts and emotions. As a remote worker, you will leverage digital tools and platforms to conduct sessions, maintain accurate records, and collaborate with a multidisciplinary team committed to the highest standards of care.

Key Responsibilities

• Deliver direct clinical services to clients, including assessments, individual therapy sessions, and family therapy, adapting approaches based on the specific needs and presenting issues of each client.
• Participate actively in team meetings to advance shared objectives, fostering collaboration and communication within the team.
• Conduct evidence-based screenings to assess the mental health needs of clients, arranging for in-depth evaluations as necessary.
• Create and implement integrated care plans, ensuring all services are appropriately linked and that clients have access to necessary resources and care.
• Monitor client assessments, including admissions, progress, and discharge criteria, ensuring timely and effective interventions.
• Engage in crisis intervention when needed, providing immediate support to clients in distress.
• Maintain compliance with the ethical guidelines and codes of conduct established by our organization.
• Ensure documentation and reporting are completed promptly and accurately for optimal client tracking and care continuity.

Patient Population / Client Focus

You will work primarily with adults who are facing severe emotional disorders and significant mental illnesses, often in the context of end-of-life care within a hospice setting. Recognizing the unique challenges of this patient population, including grief, anxiety, and existential distress, you will employ a compassionate, culturally sensitive approach to provide tailored therapeutic support.

Qualifications

The ideal candidate will possess a Bachelor's Degree in Social Work and hold a valid LCSW license. You should have a solid understanding of mental health principles, combined with experience working with clients from diverse backgrounds. Additionally, you should demonstrate effective communication skills and the ability to build rapport with both clients and colleagues.

Preferred Experience

While not mandatory, experience within a hospice or palliative care setting would be highly advantageous. Familiarity with integrated care models, proficiency in using electronic health records, and knowledge of trauma-informed care practices will enhance your effectiveness in this role.

Compensation and Benefits
