Salary: up to $97,000 / year based upon experience

$5000 sign-on bonus

Relocation bonus may be approved if needed

Location: Peru, IN

On site working M-F, 8-5 pm (no weekends, no on call, 7 paid holidays off)

Patient Population: Older adults ages 65 and above

Ideal candidate will have a healthcare background, specifically in program development, management, and community outreach 


Responsibilities:

  • Lead a small disciplinary team comprised of a psychiatric physician, therapist, nurse, and medical office coordinator providing services to the senior population.
  • Engage with your local community through education (presentations, one-on-one meetings, etc.)
  • Referral development through hospital staff, providers, and community resources
  • Growing and maintaining patient census
  • Ensure compliance through professional and ethical guidelines
  • Other duties as assigned
