Director of Nursing (DON)

We are currently seeking an exceptional Director of Nursing (DON) to join Del Rio Nursing and Rehab.  This role is ideal for a visionary nursing leader who is clinically strong, operationally savvy, and deeply committed to developing people while driving quality outcomes in a skilled nursing environment.

Key Responsibilities: Clinical & Regulatory Excellence
  • Provide strategic and day-to-day leadership of all nursing services in the community.
  • Ensure compliance with all federal, state, and local regulations, including CMS, DADS, QAPI, and infection control standards.
  • Lead initiatives that drive strong clinical outcomes, survey readiness, and sustained CMS 5-Star performance.
  • Oversee clinical risk management, quality improvement initiatives, and performance monitoring.
Leadership & Team Development
  • Build, mentor, and retain a high-performing clinical team through coaching, education, and accountability.
  • Foster a culture of trust, collaboration, and professional growth aligned with Touchstone values.
  • Serve as a hands-on educator—developing nurses at all levels through ongoing training, skills validation, and leadership development.
  • Promote staff engagement, morale, and retention through visible leadership and recognition.
Resident-Centered Care
  • Champion individualized, compassionate care that honors resident dignity, choice, and quality of life.
  • Partner with residents, families, physicians, and interdisciplinary team members to ensure optimal care planning and outcomes.
  • Lead by example in establishing meaningful connections with residents and families.
Operational Partnership
  • Collaborate with the Administrator on staffing strategies, budgeting, and resource utilization.
  • Ensure efficient scheduling, appropriate skill mix, and optimal staffing levels to support quality care and regulatory compliance.
  • Utilize data and metrics to inform decisions, improve performance, and achieve organizational goals.

What We’re Looking For: Required Qualifications
  • Active Registered Nurse (RN) license in the state of practice.
  • Minimum of 5 years of experience as a Director of Nursing in a skilled nursing facility.
  • Demonstrated success in regulatory compliance, survey management, and quality improvement initiatives.
  • Strong working knowledge of CMS 5-Star Quality Rating System, MDS, QAPI, and clinical best practices.
