The ideal candidate will be responsible for overseeing the admission process of new clients, ensuring all clinical and administrative documentation is accurate and complete, and coordinating with caregivers, clients, and healthcare providers to facilitate smooth service initiation.
Responsibilities:
  • Conduct comprehensive client assessments during the intake process.
  • Review and verify all intake documentation, medical history, and care plans.
  • Communicate with physicians, clients, and family members to ensure appropriate care coordination.
  • Maintain accurate and confidential client records in compliance with agency and state regulations.
  • Collaborate with scheduling and field staff to ensure timely assignment of services.
