Job description

Cummins Behavioral Health Systems, Inc. is seeking an experienced clinician or a recent bachelor's graduate with strong interest in helping individuals and families in a position as Integrated Health Specialist in Greencastle, Indiana. Qualified candidates should have a background in social work, human service and/or in mental health and have a passion to impact youth to older adults at our Putnam County outpatient office. 

This is a full-time position with flexibility for evening and weekend hours to assist with supervised visit coverage.

Job Summary:

The Integrated Health Specialist is a core member of the treatment team, supporting youth and adults with complex behavioral health and psychosocial needs, including individuals experiencing substance use and system involvement. This role provides trauma-informed, recovery-oriented services that promote coordinated, person-centered care across home-based, community, office, and virtual settings, and requires flexibility in delivering services across all environments. The specialist delivers psychosocial skill-building, connects individuals to essential services, and collaborates with providers and community partners. Serving as a key connector across systems, this position supports effective treatment planning, crisis prevention, and long-term recovery outcomes. 

 

Essential Duties:

  • Deliver psycho-social skill-building and behavioral management strategies to support emotional regulation, communication, problem-solving and daily living skills across home-based, community, office, and virtual setting
  • Utilize stagewise case management to support individuals and families initiating and maintaining participation in services
  • Develop and maintain person and family-centered treatment plans in collaboration with internal teams and external providers.
  • Connect individuals and families to appropriate services across multiple domains, including physical health, behavioral health, substance use treatment, housing, and financial assistance 
  • Identify and respond to safety risks by developing and implementing crisis and relapse prevention plans
  • Engage in ongoing professional development through participation in continuing education, in-service training, and supervision
  • Perform other related duties as assigned

Education and Experience:

  • Bachelor's degree in Social Work, Mental Health Counseling, Psychology, Sociology, or a related degree in human service field is required or

  • Associate's degree in human services or related field and (4) years previous direct care experience in social services, behavioral health, school-based, etc.; or

  • High school graduate or GED and four (4) years previous direct care experience in social services, behavioral health, school-based, etc; or

  • Two years' of post-bachelors work experience in mental health is preferred.
