About Us

Generations Toronto is a not-for-profit, intergenerational community campus offering affordable housing, long-term care, early childhood education, health services, and community programming. Rooted in values of pluralism, compassion, and service, Generations Toronto fosters inclusive, connected living for people of all ages and backgrounds.

The campus includes a 122-bed Ismaili Ethnocultural Designated Long-Term Care Home. AgeCare has been appointed as the manager of the Generations Toronto long-term care home and is recruiting the PSW Coordinator on behalf of Generations Toronto.

The Opportunity:

The PSW Coordinator reporting to the Director of Care works collaboratively with the senior management team to plan, organize, direct and coordinate resident personal care services within approved policies and procedures; to promote and support PSWs to provide the best care possible in accordance with AgeCare's mission and values.

What Is In It For You:

  • Works collaboratively with the Director of Care to manage the general day to day operations of the Personal Support Workers (PSWs).
  • Participates in the hiring, orientation, evaluation, coaching and mentoring of PSWs.
  • Onboarding PSWs - POC documentation, care plan/kardex, shift report and arranging PSW partnership for onboarding orientation shifts etc.
  • Monitors the care and services provided to residents through direct supervision and formal and information audits, on all shifts by way of: Leadership rounds, care audits, equipment and sling audits, meal and snack audits, hand hygiene, PPE and IPAC skill audits per schedule, analyse results and report analysis at monthly quality meetings.
  • Monitor, audit and distribution of nursing supplies, continence products, IPAC and cytotoxic supplies as per daily/weekly/monthly schedule as directed by supervisor.
  • Promote improved practices through mentorship, coaching and education.

What You Will Need:

  • Graduate of Personal Support Worker (PSW) Certificate/Diploma Program that meets the standards outlined in the Fixing Long Term Care Homes Act.

  • Must have worked as a Personal Support Worker in Long Term Care.

  • Knowledge of procedures and techniques involved in administering simple treatments and related bedside care to residents.

  • Behavioural Support Ontario (BSO) certification and Imagine training is considered an asset.

  • Strong computer literacy and typing skills, familiarity with programs such as Word, Excel, PCC, PPOC, Workday, UKG, Outlook is an asset.

  • Must understand the basics of good sanitation and clean techniques, Knowledgeable with respect to the needs of geriatric residents.

  • Must be mature with effective communication (both written and verbal) and interpersonal skills.

Salary Range: $45,000.00 - $55,000.00 per yearSalary placement based on qualifications and experience.
