How much do direct primary care jobs pay per hour?

As of Jun 9, 2026, the average hourly pay for direct primary care in the United States is $16.77, according to ZipRecruiter salary data. Most workers in this role earn between $14.90 and $17.79 per hour, depending on experience, location, and employer.

What is Direct Primary Care?

Direct Primary Care (DPC) is a healthcare model where patients pay their primary care providers a flat monthly or annual fee directly, instead of using traditional health insurance for routine services. This arrangement typically covers most primary care services, including check-ups, preventive care, and basic lab work, without additional co-pays or hidden costs. DPC aims to improve patient access to care, foster stronger doctor-patient relationships, and reduce administrative burdens for physicians. Patients often benefit from longer appointments, more personalized care, and easier access to their doctors through phone or email. However, DPC does not usually replace the need for insurance to cover specialist care, hospitalizations, or emergency services.

What is the difference between Direct Primary Care vs Family Physician?

AspectDirect Primary CareFamily Physician
CredentialsMedical degree, state medical license, often no additional certification requiredMedical degree, state medical license, board certification in family medicine
Work EnvironmentPrivate clinics, concierge practices, direct patient relationshipsHospitals, clinics, private practices, broader patient population
Employer & Industry UsageIndependent practices, direct-to-patient modelHospitals, healthcare systems, private practices
Comparison Search IntentUnderstanding alternative primary care modelsTraditional primary care roles

Direct Primary Care providers focus on personalized, membership-based healthcare without insurance billing, often working in private clinics. Family Physicians are licensed medical doctors providing comprehensive care, typically within hospitals or clinics, and may work in traditional or concierge settings. While both roles involve primary care, DPC emphasizes direct patient relationships and subscription models, whereas Family Physicians follow broader medical protocols within the healthcare system.

What are some of the unique challenges faced by physicians working in Direct Primary Care (DPC) practices?

Physicians in Direct Primary Care often face the challenge of building and maintaining a patient panel, as practice growth relies heavily on word-of-mouth and community engagement rather than traditional insurance referrals. Additionally, DPC doctors must handle a broad range of administrative and business responsibilities, from marketing the practice to managing memberships and billing directly. However, they typically benefit from more time with patients and less insurance paperwork, allowing for deeper patient relationships and personalized care. Collaboration with external specialists and hospitals is still essential, requiring strong communication skills to coordinate patient care outside the DPC model.

What are the key skills and qualifications needed to thrive as a Direct Primary Care physician, and why are they important?

To thrive as a Direct Primary Care (DPC) physician, you need a medical degree, board certification in family or internal medicine, and strong clinical diagnostic skills. Familiarity with electronic health records (EHRs), telemedicine platforms, and practice management systems is typically required. Excellent communication, empathy, and an entrepreneurial mindset help build lasting patient relationships and manage a patient-focused business. These skills ensure high-quality, accessible care while supporting the unique, patient-centered model of the DPC practice.

Job description

Description:

ABOUT PEOPLEONE HEALTH:

PeopleOne Health is one of the fastest-growing providers of value-based primary care, consistently earning best-in-class member satisfaction scores. We deliver exceptional healthcare that lowers costs and significantly improves health outcomes by emphasizing preventive care, behavior change, and a proactive approach to wellness through our unique direct primary care model. Our culture thrives on a simple yet powerful motto: care for yourself, care for each other, and care for our memers.

JOB SUMMARY:

PeopleOne Health is seeking a board-certified Primary Care Physician with training in Family Medicine or Internal Medicine/Pediatrics to join our team. We are looking for a dynamic physician who is passionate about providing full-spectrum primary careโ€”including care for both adults and childrenโ€”and dedicated to building strong, long-term relationships with patients. The ideal candidate shares our commitment to helping individuals make informed health decisions by connecting them with resources that support healthier lifestyles and improved outcomes.

At PeopleOne Health, physicians are empowered to practice at the top of their licensure. This includes performing common in-office procedures such as joint injections, skin biopsies, cryotherapy, laceration repair, incision and drainage, and women's health procedures such as Pap smearsโ€”ensuring comprehensive, accessible care within the primary care setting.

Our care model removes the constraints of traditional, insurance-based primary care, allowing you to focus on what matters mostโ€”your patients. With smaller patient panels, longer appointment times, and significantly fewer administrative burdens, youโ€™ll have the time and support needed to practice medicine the way it was meant to be. Youโ€™ll work alongside a multidisciplinary team that includes an onsite clinical pharmacist, registered dietitian, and care coordinators, all focused on delivering exceptional care and service.

The perfect candidate is a motivated, self-directed clinician who takes pride in educating, healing, and advocating for their patients. At PeopleOne Health, we are focused on members, not numbers. Our patients benefit from extended visits, coordinated care, and access to services such as mental health screenings and specialist consultations through eConsult platformโ€”all delivered from within the health center and with the support of their trusted physician. If you are seeking a more personalized, relationship-centered way to practice medicine, we invite you to explore this opportunity to be part of something truly transformative.
