Description:
The Medical Housing Navigator supports shelter participants with complex medical, disability, and aging-related needs by coordinating assessments, medical care, and placement into Medicaid-funded housing options, including assisted living and care facilities. This role balances direct client engagement, housing navigation, external systems coordination, and administrative
documentation to ensure timely, ethical, and sustainable housing outcomes.
Housing Navigation and Case Management
- Support clients with housing navigation, including tours and applications to Medicaid-funded housing
- Coordinate and attend client appointments related to medical or housing needs
- Assist with coordinating or purchasing medical supplies and durable medical equipment
- Provide ongoing client check-ins and case management support
- Complete timely case notes in Charity Tracker
Assisted Living and Care Facility Outreach
- Visit Assisted Living or care facilities
- Build and maintain working relationships with facility staff
- Track facility requirements, referral processes, and availability
- Maintain and update a running list of visited facilities
DHS Assessments and Client Appointments
- Coordinate and attend DHS and Long-Term Care assessments
- Support clients with preparation and documentation
- Communicate with DHS caseworkers regarding outcomes
- Track assessment status and follow-up actions
Weekly schedule may flex based on client acuity, appointment availability, and emergent needs.
Requirements:
Preferred qualifications
- General knowledge and understanding of harm reduction along with utilizing trauma informed care skills.
- Willing and able to serve our community with compassion,
- Experience in medical housing navigation.
- Experience navigating social services, in general, is preferred.
