We are seeking a dedicated Hospice Registered Nurse to provide compassionate hospice care to patients throughout King and S. Snohomish Counties. Care is delivered across a variety of settings, including private homes, adult family homes, assisted living communities, and skilled nursing facilities. In this role, you will coordinate and manage end-of-life care by addressing immediate and ongoing patient needs, managing symptoms, and ensuring each patient receives high-quality, dignified care that aligns with their goals, values, and wishes.

Primary Responsibilities
  • Perform initial and ongoing comprehensive assessments of patients' physical, emotional, and spiritual needs and develop individualized plans of care.
  • Collaborate with physicians, social workers, volunteers, and interdisciplinary team members to implement, coordinate, and evaluate care plans.
  • Administer medications and therapeutic interventions to effectively manage pain and symptoms, promoting comfort and quality of life.
  • Educate patients and families on the hospice philosophy, disease progression, symptom management, self-care, and end-of-life expectations.
  • Provide emotional support and counseling to patients and families experiencing grief, anxiety, or other psychosocial concerns.
  • Maintain accurate, timely, and compliant clinical documentation reflecting care provided and changes in patient condition.
  • Ensure care delivery complies with Medicare guidelines, state regulations, and hospice policies and standards.
  • Advocate for patient dignity, preferences, autonomy, and rights in all aspects of care.
  • Respond promptly to urgent patient needs and collaborate with the on-call team as necessary.
  • Participate in quality assurance, performance improvement, and interdisciplinary team meetings.
  • Participate in scheduled weekend rotations
Requirements
  • Current, active Washington State RN license in good standing
  • Active BLS certification
  • Valid driver's license and reliable transportation
  • Minimum 1 year nursing experience in hospice or home health preferred
  • Minimum 2-3 years of healthcare related experience required
  • Proficient with EMR systems and healthcare technologies
  • Ability to exercise sound judgment, initiative, and work independently within scope of practice
  • Adaptable to diverse social, cultural, and religious backgrounds
