This role is based in a remote setting, allowing flexibility while working with seniors across various locations. You will become part of an interdisciplinary team that includes nurses, caregivers, and other professionals, working together to provide holistic care tailored to our clients' needs.

• Conduct thorough assessments of seniors’ psychosocial needs, considering emotional, mental, and social factors.
• Collaborate with clients and their families to develop personalized care plans that address specific goals and preferences.
• Provide counseling and emotional support to seniors and their families, assisting them through life transitions and challenges.
• Advocate for clients’ rights and access to necessary community resources, ensuring they receive the best available support.
• Coordinate with interdisciplinary teams, including nurses and caregivers, to deliver cohesive and comprehensive care.
• Perform regular check-ins with clients to monitor well-being, assess living conditions, and tackle any arising concerns.
• Facilitate communication among family members involved in the senior client’s care, resolving conflicts when necessary.
• Educate clients and their families about community resources, support groups, and engagement opportunities to foster social connections.
• Maintain accurate documentation of client assessments, care plans, and progress notes as per industry best practices.
• Stay updated on current social work practices and regulations to ensure compliance and uphold ethical standards.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of hands-on experience in a social work role, ideally within geriatric or community health settings.

• Exceptional assessment, counseling, and problem-solving skills tailored to senior care.
