Family Radiance Counseling is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team in a remote capacity. This remote position allows flexibility in daily operations, making it easier to connect with clients through various digital platforms. Key working hours are Monday through Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs. • Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, taking into account various emotional, mental, and social factors.
• Collaborate closely with seniors and their families to create individualized care plans that reflect their unique preferences and goals for support.
• Provide counseling and emotional support to clients, assisting them in navigating life's transitions, such as health crises or relocation.
• Advocate for clients to ensure their rights are upheld and they have access to necessary community resources, such as housing assistance and healthcare services.
• Regularly monitor client well-being through virtual visits, evaluating their living conditions and making adjustments to care plans as required.
• Facilitate effective communication among family members, addressing conflicts and ensuring that everyone is aligned in providing care.
• Educate clients and families about available community resources and social engagement programs to promote mental, emotional, and social well-being.
• Maintain meticulous documentation of client assessments, care plans, progress notes, and other interactions to ensure continuity and quality of care.
• Stay current on best practices in social work, continuing education requirements, and relevant regulations to ensure compliance with ethical standards.

Your work will focus primarily on seniors, a demographic that faces unique emotional and physical challenges. You will be instrumental in helping them navigate health concerns, family dynamics, and social isolation, while ensuring they receive tailored support and resources to live fulfilling lives.

• A Bachelor’s Degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.
• A minimum of two years of hands-on experience as a social worker, preferably within a community or geriatric setting.
• In-depth knowledge of social work principles, particularly as they relate to aging and the available resources for senior populations.
• Exceptional assessment, counseling, and collaborative problem-solving skills.
• Ability to demonstrate compassion, empathy, and rapport-building with seniors and their families.
• Strong verbal and written communication skills, facilitating effective teamwork with interdisciplinary professionals.
• Proficient in electronic health record (EHR) systems and related software used in clinical settings.

• Experience working in a community health clinic or a non-profit environment focused on senior care.
• Familiarity with local resources and social services specific to the Olathe, Kansas area.
• Proven ability to work effectively in a remote environment while maintaining high-quality client engagement.
