Job description
For more than55 years, Family Health Centers of San Diego's (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons.
FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy.
Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women's care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management.
The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region.
Family Health Centers of San Diego is looking for a Supervisor, Digital Health Program who supervises employees and day-to-day processes associated with Family Health Centers of San Diego's digital health programs. Ensures efficient delivery of health education topics, supervises digital health technology orientation, technical assistance, and deployment for our patients across multiple projects and quality improvement and innovation initiatives. Supervises provision of complementary patient navigation of FHCSD resources and services to enhance access to care, utilization of the patient portal, and uptake of educational resources. Supports the mission of FHCSD by driving the development and offering of culturally and linguistically appropriate education, navigation, and technical guidance for the utilization of digital health technology.
Responsibilities:
- Builds positive relationships, inspires others to perform at a higher level, prepares career development plans and retains an accountable workforce.
- Coaches and counsels' staff and administers appropriate disciplinary action in a timely manner.
- Creates and implements systems and processes to ensure company goals and strategic plans are met, including evaluation, recommendation of changes, implementation, training, and evaluation of effectiveness.
- Collects and disseminates training materials, conducts trainings, and provides on-going supervision for digital health specialist team.
- Ensures compliance with all regulatory requirements: HEDIS, Meaningful Use, Joint Commission, State Licensing, NCQA-PCMH, and other entities governing care delivery and quality.
- Participates in continuous improvement activities and ensures appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.
- Provides/ensures staff receive all new hire and annual trainings on time.
- Supervises assigned team, including hiring, providing performance management, training, scheduling, and corrective discipline (including termination) as needed.
- Works closely with interdisciplinary teams to develop and implement digital patient navigation and education programs to improve patient knowledge and compliance.
- Supervises Digital Health Specialists activities, which include assessing potential barriers to care and identification of supportive steps to overcome these barriers, assisted and unassisted referrals, and providing resources and follow-up. As needed, may provide direct support to patients in support of team.
- Tracks expected and projected deliverables on a regular basis and ensure compliance with all regulatory and departmental requirements. Oversees the development of and ensures correct material is delivered in culturally appropriate and relevant ways.
- May engage patients in ongoing support for digital health technology utilization and provides guidance to patients in need of technical assistance.
- Leads assigned projects by developing project plans.
- Audits project plans on an ongoing basis and provides reports to the manager.
- Ensures adherence to master plans and schedules and generates solutions to program problems.
- Performs other duties as assigned.
Requirements:
- Master's degree in Health Education, Health Science, Nutritional Science, Public Health, Communications, or closely related field preferred.
- 3 years of experience delivering health education/healthcare, preferably in a community health setting.
- Specific Certifications may be required or preferred depending on assignment (i.e., CA HIV Test Counselor, limited CPT certification for waived testing, lactation certification, etc.)
- Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver's license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Mileage and other reimbursement governed by policy.
Experience/Specialized skills (including Language)
- Ability to conduct research and investigate latest trends and best practices on health education topics.
- Ability to work effectively with patients and community members from diverse social, cultural, and economic groups.
- Ability to work well in both a team-based environment and independently.
- Intermediate to advanced computer literacy ability to comply with department needs and expectations (i.e., digital health device familiarity, electronic medical record documentation, obtaining background information and reports on patients, following up on appts, etc.).
- Bilingual in Spanish/English or other language may be required depending on assignment.
- Demonstrated ability to be culturally sensitive and respect diversity.
- Good written and verbal communication skills.
- Knowledge of adult and child education techniques, practices, and tools.
- Knowledge of current health science theories and best practices, and ability to translate best practices into meaningful, culturally appropriate resources.
- Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners.
- Strong interpersonal, customer service and supervisory skills.
