Job description
Position Summary
The coordinator is responsible for program activities under the direction of the Nurse Case Manager. Activities include, but are not limited to, recruitment, screening, retaining, training, instructing, coaching, and counseling program participants. The employee will serve as the fitness specialist, exercise leader, and provide program support for compliance and monitoring. The incumbent will have a shared responsibility for data collection and monitoring participants. They win also monitor for health risk and refer to KNHC for further evaluation and/or medical assistance.
They will ensure that program goals are met in a timely manner and in compliance with DIIHS/PHS/IHS and their respective health facility policies. The incumbent will attend meetings deemed required by grant and needed training travel will be required.
Major Duties:
Responsible leading the diabetes prevention education in the implementation of the intensive lifestyle education component of the SDPI Diabetes Prevention Program Competitive Grant using the standardized curriculum.
Will work with participants and family members to provided nutrition education as outlined in the DPP curriculum and will work with participants as a lifestyle coach to implement the nutrition guidelines presented.
Will work closely with participants and family members to identify ways to increase exercise and to develop exercise strategies to meet exercise goals. The Lifestyle Coach will help project participants to achieve specific weight loss behavior change goals as outlined in the standardized curriculum.
Will function as an exercise specialist, planning, supervising, and teaching exercise classes at the tribal wellness site.
Responsible for planning and reporting on less-intensive activities such as overseeing the fitness center, organizing Fun Runs and Softball Tournaments, other activities as determined to be appropriate and feasible grant activities.
Assist with the behavioral reinforcement and incentive components of the project for participants.
Document all relevant participant data for the study participants.
Assist in the development and monitoring of all performance improvement initiatives and reporting requirements of the grant.
Participate in local and national team meetings
At the discretion of the supervisor, other duties not specifically described may be required from time to time to meet departmental objectives and obligations, or to improve the level or quality of services provided by the incumbent.
Desirable Education Recommended for the Position:
A Bachelor's Degree is preferred. Course work should include a minimum of 9 hours in Nutrition and/or health/wellness related coursework.
LPN or RN certified.
CPR certification will be required prior to teaching any exercise classes
Knowledge Required by the Position:
Knowledge of basic nutrition principles.
Knowledge related to the role exercise, physical activity and nutrition play in the prevention and treatment of diabetes.
Knowledge of exercise leadership
Knowledge of educational concepts to promote behavior change.
Knowledge of basic medical terminology to record and report medical information
Knowledge of proper grammar, spelling, and punctuation for purposes of documenting observations and recordings used in the provision of services.
Knowledge of the regulations on confidentiality and patient eligibility.
Familiarity with community resources
Ability to utilize a variety of computer software and applications.
Ability to plan, coordinate, schedule and evaluate program activities within communities.
Ability to communicate courteously and effectively with patients, their families, medical and administrative staff, health care professionals, and the general public.
Ability to communicate both verbally and in writing
Ability to use a variety of audio-visual equipment
Ability to demonstrate safe and proper use of exercise equipment
Supervisory controls:
The Project Director, Project Coordinator, and CGP Coordinating Center staff define the objectives, priorities, and deadlines for projects or assignments and assist the employee with unusual situations or problems that do not have a clear precedents. Work reviewed by supervisor (Nurse Case Manager/ Site Coordinator) for overall program effectiveness in meeting grant objectives.
Guidelines:
Guidelines include Kickapoo Nation Health Center policies and regulations; Tribal, State and Federal laws and policies; RPMS/PCC manuals, Privacy Act Guidelines/HIPPA regulations; discussions with the Director and Coordinator of the Grant. Other guidelines include Standards of Care for Pre-Diabetes and others as directed by Grants Management and the CGP Coordinating Center. Guidelines are applicable in most, but not every situation. The employee uses judgment to select the guideline that is appropriate for the specific situation. Problems are to be referred to the supervisor or other delegated authority in charge when existing guidelines cannot be applied.
Complexity:
Work will involve a variety of processes and methods. Decisions are based upon the employee's knowledge of duties, priorities, policies, and program goals of the specific area of performance.
Scope and Effect:
The various efforts expended by the employee have a direct and critical impact on the provision of quality services to the patient population served. Competency and accuracy affect patient and program outcomes. The goal is the enhancement and promotion of the health and wellbeing of patients consistent with program goals and quality of life outcomes.
Personal Contacts:
Contacts will be with participants and their families, community groups, other health care professionals within the service unit, grantees nationwide, and CGP Coordinating Center staff.

Physical Demands:
This position requires the ability to lift 25-50 lbs. This position requires operation of a motor vehicle for travel to various community-meeting places. A current driver's license is required. Ability to demonstrate the range of motion required for different exercises is required.
Some of the work is sedentary, involving planning/coordinating of grant activities. Work Environment:
Work is performed in the Kickapoo Nation Health Center - Diabetes Prevention Program at a variety of community buildings and settings, in clinics, in homes, and in the office. Potential health hazards encountered during the provision of services may include exposure to infectious disease agents, and interaction with clientele in stressful situations.
