Speech Therapist – Hospital at Home Program (Weston FL)

Activa Home Health is expanding its Hospital at Home program, delivering hospital-level care to patients in their homes as an alternative to inpatient hospitalization. Patients remain formally admitted to a hospital partner while receiving coordinated in-home care supported by physician oversight and continuous clinical monitoring.

Provide hospital-level skilled nursing care to patients enrolled in a Hospital at Home program. SLPs deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky).

Paid Orientation

Mileage reimbursement, per signed agreement

Provide hospital level skilled speech therapy services in patients' homes

Evaluate articulation & speech sound production, language comprehension, cognitive, social communication, oral motor skills, and feeding, swallowing

Develop and implement personalized treatment plans based on physician orders

Educate patients and caregivers to support therapy, goals, and communication strategies

Accurately document visits and progress in the EMR system

Monitor progress, document outcomes, and adjust treatment as needed

Collaborate with an interdisciplinary care team to ensure coordinated, high quality care

Comply with all agency policies, and applicable state and federal regulations

Active Florida SLP license in good standing

Degree from an accredited Speech Therapy program

Valid driver's license & reliable transportation

Current CPR certification

Excellent communication, documentation, and time-management skills

Level two background screening

Recent negative TB test or chest x-ray

Strong, clinical knowledge in speech, language, pathology practice

1 year + experience in multidisciplinary setting, preferred

Home health & Hospital experience, preferred
