Company Description

Job Description

1.       Overseeing the services and supports provided to participants

              a.Monitoring and participating in the implementation of the ISP

              b. Supervising the delivery of services and ensures strategies reflect the goals of the ISP

              c.   To monitor the progress towards the achievement of the ISP goals and make recommendation

2.The DDP will supervise the formulation of the participant's plan for delivery of all waiver services  provided to the participant by the provider.

3. Conduction of functional assessments to support formulation of the participant's plan for delivery of all waiver services that include:

               a. Health Risk Screening Tool

               b.Support Intensity Scale

               c.Functional Behavioral Analysis

               d.and all other required assessments

4.  Supervising all high intensity services that address health and safety risks for the participants.

5.  Interview and Hire prospective new staff as needed

6. Provide leadership, support, and direction to staff through

               a. Role modeling appropriate treatment toward residents

               b.Communicating essential information to staff

               c.Responding to follow up and concerns of staff

               d. Staff meetings as necessary

7. Assist in coordination of home/facility schedule, as well as handling schedule changes as they occur.

8. Initiate and implement disciplinary actions to correct staff performance as necessary.

9. Complete written evaluations for staff

10.Be familiar with goals/programs for individuals residents. Monitor staff implementation of resident  programs, ensuring completion.

11.Participate in team meeting as assigned

12. Intervene in crisis situations using approved methods, promoting the safety of everyone involved.

13.Be familiar with appropriate agency, DBHDD,CQL,DCH standards for residential facilities , ensuring maintaining of these standards.

14. Provide direct supervision of staff according to agency or program policies and procedures as directed  by Executive Director.

15.Successful completion of all required trainings as identified in the DBHDD Community Standards Policies for all employees and DDP. And other trainings assigned by the agency and supervisor in a  timely manner.

Qualifications

Bachelor's degree in Social Work, Psychology, Counseling, Healthcare Management, Behavioral Health, Human Services, or related field from a four-year college or university.

Bachelor's Degree in Education from an accredited program that includes a concentration in Special Education in college coursework AND Teaching Certificate in Special Education, AND one (1) year of classroom experience in teaching individuals with intellectual/developmental disabilities.

Minimum of one year experience working in developmental disability services.

Minimum of one year working in an administrative office position.

Strong customer service and problem solving skills.

Excellent verbal and written communication skills.

Excellent organizational skills.

Proficiency in Microsoft Office Professional, Outlook, and Web based systems.

Positive, upbeat attitude.

Accurate file keeping.

Accurate data entry and ability to meet report deadlines.
