Social Services Director / PRSD

Job Summary:

We are seeking a highly skilled and compassionate Social Services Director to join our team at our Skilled Nursing Facility in Oak Park, IL. As a Social Services Director, you will be responsible for planning, directing, and evaluating the social services program, ensuring that our customers receive the highest quality of care and support. Responsibilities:

  • Plan, direct, and evaluate the social services program, including social services staff, direct services, advocacy, counseling, service coordination, education, in-servicing, policy, and program development, and community relations.

  • Participate in an interdisciplinary team approach to better serve the needs of our nursing center population.

  • Provide therapeutic intervention to help customers cope with social and psychological aspects of their illnesses, impairments, or disabilities.

  • Maintain written documentation in the customer medical record per company policy.

  • Develop social service components of the customer's plan of care, identifying specific problems, goals, and approaches.

  • Coordinate discharge planning, including the development of an organized discharge plan for all customers.

  • Ensure the safety of all center customers, adhering to legal, safety, health, fire, and sanitation codes.

Requirements:

  • Master's Degree in Social Work required.

  • Possesses any certified/licensed required by State regulations.

  • Ability to communicate effectively and appropriately both verbally and in writing in a wide variety of circumstances.

  • Commitment to philosophy and objectives of the Skilled Nursing Facility.

  • Knowledge of governmental regulations relating to social service delivery.

  • Ability to work in a fast-paced environment with multiple priorities.
