Job description

Overview
The Designated Coordinator role executes the agency's person-centered approach, ensuring all plans and support strategies meet the unique needs, preferences, and goals of each individual. Amivie provides essential community-based services to adults with intellectual and mental health disabilities, promoting independence, inclusion, and a high quality of life. The position requires a strong grasp of person-centered planning principles and the ability to apply them to daily operations.
Responsibilities
  • Facilitate and/or complete client intakes, incorporating a person-centered served plan with team members.
  • Develop, write, and implement each client's person-centered support plans, including the Coordinated Service and Support Plan (CSSP), Self-Management Assessment (SMA), and Individual Abuse Prevention Plan (IAPP).
  • Ensure and maintain all required signatures are maintained in accordance with 245-D licensing rules and regulations.
  • Maintain and update all client files as mandated by 245-D licensing rules and regulations.
  • Ensure full compliance with 245-D licensing rules and regulations.
  • Prepare quarterly and annual reports as required or directed by the person served and/or the team to fulfill the person-centered plan.
  • Demonstrate the ability to problem-solve effectively and creatively as needed.
  • Coordinate and ensure the protection from abuse and the general well-being of clients.
  • Provide staff orientation, training, and supervision regarding person-centered needs and services as documented in the client's specific plan.
  • The designated coordinator is responsible for monitoring target symptoms, attending psychiatric appointments, and maintaining communication with all team members.
  • Be available to receive calls from staff during and outside of regular work hours for consultation or in emergency situations.
  • Must be able to perform physical demands of the position (i.e. lifting, transfers).
  • Other duties as assigned.

Job Type:
  • The standard working hours for this position are Monday through Friday, 8:00 a.m. to 5:00 p.m.
  • Additional hours outside of the standard schedule may be necessary, contingent upon client needs.
  • All offers of employment are conditional upon a satisfactory background check.

Qualifications
  • Associate's degree required.
  • Fluent in English is required.
  • Ability to pass a background check administered by the Department of Human Services (DHS) is required.
  • Must meet the qualifications of a Designated Coordinator under 245D licensing regulations.
  • A current and valid driver's license is mandatory.
  • Consistent and reliable access to a vehicle is essential.
  • A satisfactory driving record is required.
  • One year of supervisory experience is highly preferred.
  • Demonstrated experience working with individuals with Mental Health challenges: 2 years (Preferred)
  • Demonstrated experience working with individuals with Developmental Disabilities: 1 year (Preferred)
