JOB SUMMARY:

An in-home care Registered Nurse (RN) will help assess and formulate individualized care plans for clients while directing care staff on day to day needs to maximize quality life. 


RESPONSIBILITIES:
  • Manage a caseload of clients assigned by supervisor
  • Provide oversite to client care to clients with varying backgrounds and medical histories
  • Oversee safe medication administration practices 
  • Complete required documentation timely and accurately
  • Assess clients for change ofcondition
  • Keep clients family andphysician informed of any changes in client behavior, medications, orconditions of health
  • Assign and provide instruction for home health aides
  • Communicate needs of residentto family and outside providers
  • Comply with HIPPA regulations in an out of the office
  • Other Duties as Assigned

REQUIREMENTS: Registered Nurse (RN) Licensure in the State of practice
