Job description

Job Summary and Responsibilities

As ourGeriatrician, you will helpolder adults and patients with dementia or palliative care needsreceive specialized, comprehensive medical care and compassionate supportso they canimprove their health outcomes, maintain dignity, and enhance their quality of life.

In this role, you willprovide direct patient care in a clinic setting, focusing on geriatrics, dementia screening, and palliative care.You will be expected tocollaborate with interdisciplinary teams and Advanced Practice Providers, engage in patient education and shared decision-making, and contribute to innovative programs like the Dementia Screening Clinic and the GUIDE model.

To be successful in this role, you must board certified or board eligible, be a skilled Geriatrician with a passion for caring for older adults, expertise in dementia diagnosis and palliative care, and the ability to work collaboratively within a dynamic, multidisciplinary team.

  • Patient population includes Geriatrics, Dementia, and Palliative Care
  • Collaborate closely with interdisciplinary teams and Advance Practice Providers to enhance efficiency and care coordination to ensure comprehensive patient care
  • Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
  • Serve as a Geriatrics Consultant, including screening and diagnosing Dementia
  • Clinic position with possibility of home visits
  • Full time position, Monday through Friday
  • Provider will have 4 hours of administrative time per week
  • Daily patient volume: anticipated 6-10 patients per day
  • Current programming includes multi-disciplinary Dementia Screening Clinic, Alzheimer's & Dementia Care Program (UCLA dissemination site), Outpatient and Inpatient Palliative Care, and GUIDE model participant
  • Opportunity to enhance and expand services in the Cunningham Center for Compassionate Aging

Job Requirements

Required

  • BE/BC in Geriatric Medicine (IM or FP), Neurology, or Hospice & Palliative Medicine (IM or FP)
  • Experience or interest in working with Dementia/Alzheimer's Disease population
  • Strong communication and interpersonal skills
  • Active or eligible license in the state of Arizona
  • Valid DEA certificate
  • Strong clinical skills and experience working with older adults
  • Ability to work effectively in a team-based healthcare environment
  • Demonstrate empathy, compassion, and respect for the unique needs of older adults and their care partners
