Job Summary

We are seeking a Nurse Practitioner to join our growing skilled nursing facility practice. This role is well-suited for early- to mid-career clinicians looking for meaningful work, flexible scheduling, and a collaborative clinical environment.
You will provide comprehensive medical care to residents in skilled nursing facilities, work closely with interdisciplinary teams, and contribute to high-quality, patient-centered care. This position offers autonomy, consistency, and the opportunity to build strong clinical relationships.

Responsibilities – Skilled Nursing Facility Provider

  • Provide assessment, diagnosis, and management of acute and chronic conditions
  • Conduct regular patient rounding and timely evaluations
  • Order and interpret labs, diagnostics, and imaging
  • Prescribe and manage medications in compliance with regulations
  • Maintain accurate, timely documentation in the EMR
  • Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams
  • Respond to change-in-condition and urgent care needs
  • Support advance care planning and end-of-life discussions
  • Participate in quality assurance and regulatory readiness efforts
Qualifications

  • Active Pennsylvania Nurse Practitioner license and required certifications
  • Graduation from an accredited Nurse Practitioner program
