Attending Physician - Skilled Nursing Facility (Granville, NY)
$150,000-$300,000+ per year. Granville, ny • Full-time • Physician-owned group • RVU eligible
Medical Director Services PC (MDS PC) is hiring an Attending Physician (MD/DO) to care for residents at a skilled nursing facility in Granville, NY. Founded in 2016, MDS PC is a physician-led group built specifically for the SNF setting - we focus on treating patients in place, reducing avoidable hospitalizations, and keeping providers closely connected to the surrounding hospital network for continuity of care.

How our teams work: Our APPs generally handle day-to-day routine and acute care on the units, which allows our physicians to expand their census, increase your RVU-generating opportunities, and take on additional responsibilities over time - including the potential for Medical Directorships.
What you'll do:
  • Perform admissions, monthly, acute & routine visits for SNF residents, on a regulatory and as-needed basis.
  • Collaborate with APPs on your units
  • Manage medication decisions - including narcotics and psychiatric medications - with an emphasis on reducing unnecessary polypharmacy
  • Lead care planning, discharge planning, and ongoing staff education
  • Support the facility through regulatory and survey site visits
  • Coordinate with local hospitals to ensure continuity of care for residents
What you'll need:
  • Active NYS medical license (MD or DO)
  • Current DEA registration in good standing
  • Up to date on CME requirements
  • Experience in SNF, long-term care, or managed care settings preferred
  • Comfort working with a culturally and linguistically diverse resident population
