Assistant Program Manager responsibilities:

  • Provide direct and personal care, and support with activities of daily living to our people served
  • Maintain an environment that is safe, clean, and comfortable. Including but not limited to: fixing or reporting damaged items, maintaining a household cleaning schedule and vehicle maintenance, meal planning, overseeing medical equipment, and more.
  • Managing staff by supervising shifts, providing input on performance reviews and coaching/disciplinary actions, providing leadership, training, and support.
  • Track individual’s medical needs and appointment calendars, scheduling appointments, accompanying individuals to their appointments, ordering medications, ordering medical supplies, etc.
  • Engage in proactive communication with people receiving services, coworkers, families, work/day programs, case managers and others.
  • Signing individuals up for activities that they are interested in, keeping the program calendar up to date, communicating with team.
  • Manage Person Centered Planning/Coordinated Service and Support Plan or CSSPA process and paperwork for people receiving services.
  • Complete other tasks and responsibilities as assigned.

Qualifications

  • Must be 18 years or older
  • Valid Driver’s License
  • Pass a background check through MN Department of Human Services
  • Two years relevant post-secondary education or equivalent experience preferred
  • Technical skills – must be comfortable and proficient in using computers and/or smart technology
  • Prior leadership experience preferred as well as experience coordinating schedules
  • Ability to lift and/or move up to 30 pounds regularly and 100 pounds occasionally

We provide 24 hours services, 7 days a week and have a variety of different full time and part schedules available! Most employees work a combination of some week and weekend hours.

Living Well provides services in 36 homes and In-Home Support cross several different counties:

  • Hennepin: Brooklyn Park, Brooklyn Center, Edina, Maple Grove, Crystal, Plymouth, Long Lake, Golden Valley, Richfield
  • Dakota: Eagan, Apple Valley, Burnsville, Inver Grove Heights, Mendota Heights, West St. Paul, Hastings, Rosemount
  • Ramsey: Maplewood and Little Canada
  • Isanti: Cambridge
  • Sherburne: Big Lake
  • Wright: Buffalo, Albertville
