Job description

Position: Day Program Provider:

Programs: DTA (Adult Day Program-Full Time)/ DTT (Teen Program-Part Time)

Schedule / Location: Varies by Program

Core Responsibilities:

  • Facilitate daily activities, structured programming, and community outings
  • Support living skills, personal hygiene, feeding, and behavior management
  • Implement individual DDD goals
  • Encourage participation and positive social interactions
  • Monitor members' health and well-being
  • Complete daily documentation and data sheets accurately
  • Follow all safety, emergency, and health protocols (including medication administration if authorized).
  • Support members during transportation and trasitions
  • Model appropriate community behavior and safety awareness at all times
  • Serve as a positive role model to members and co-workers
  • Follow chain of command and consult group leads when needed

Facility Duties:

  • Complete daily cleaning and chore assignments
  • Assist with decorating for holidays and special events
  • Communicate new activity ideas
  • Cooking(once per week)

  • Participating in occasional Respite Nights and special events (if applicable)

  • Dress Code: Maintains a professional appearance by adhering to all agency dress code standards

Qualifications:

  • Level 1 Fingerprint Clearance Card
  • Criminal History (background check)
  • Valid AZ Drivers License
  • Driver's Motor Vehicle Report (MVR)
  • CPR & First Aid Certification
  • Article 9
  • DCW (Direct Care Worker) Certification
  • Prevention and Support Certification (within first 90-day of emplyment)
  • Abuse and Neglect (within the first 90-day of employment)
  • 18 years of age or older
  • 21 years of age or older to be a driver
