Habilitation Assistant

United Cerebral Palsy Associations of New York State, Inc. d/b/a Constructive Partnerships Unlimited (CP Unlimited) is one of New York City's largest and most wide spread Community Living Organizations with programs dedicated to enhancing the lives of people with intellectual and developmental disabilities. CP Unlimited is a broad based multi-service organization providing services and programs for people with developmental disabilities.

In the NYC metropolitan area, the agency operates an extensive community based living program, day habilitation programs, and community habilitation services. CP Unlimited focuses on giving people the personal, social, and vocational supports needed to live in their communities. CP Unlimited is currently seeking several Habilitation Assistants to join our team of professionals at our Queens Long Island City Day Program. The positions are full-time 8:30am-4:30pm and are 35 hours weekly Monday-Friday.

In this vital position the Habilitation Assistant will monitor the activities of the people who are attending the day program and ensure the activities are consistent with valued outcomes of independence, individualization and productivity. The Habilitation Assistant will ensure the activities and training provided to the people attending our program will maximize their independence through a person centered planning approach.

The Habilitation Assistant will implement community based training opportunities, which includes use of public transportation, travel training and safety skills. Ensures daily documentation is maintained and uses various software for data collection. Will directly work with the people who attend the program and assist with feeding, personal hygiene (including toileting as needed) and lifting/transferring as needed. The Habilitation Assistant will ensure the health, safety and wellbeing of all the people that attend our program.

Will also ensure the maintenance of equipment used at the Day Program and ensure the Day Program is clean, safe and all concerns are reported to management. The Habilitation Assistant will notify management and/or nurse of any medical concerns of the people attending the program and accompany them to the hospital and/or health care provider as need.

To qualify for the Habilitation Assistant position you must have the following: High School Diploma and/or GED from an accredited school. 1 year experience working with people with intellectual and/or developmental disabilities. Excellent communication and interpersonal skills. Must be willing to work outdoors for community based activities as needed. Must be able to perform data entry on a personal computer (PC). MS Word a plus. Criminal background check. Valid NYS Driver's License preferred. Will be required to successfully pass a Medication Administration Program.
