Cardiovascular Physicians of Delaware

Licensed Practical Nurse (LPN)- Coumadin Coordinator

Location- Lewes and Millville, DE

Schedule- Monday-Friday, dayshift


Job Summary

The LPN’s primary role is coordinating Cardiovascular Physicians of Delaware's Coumadin clinic. This entails responsibility for overseeing all patients of the practice who are currently on Warfarin/Coumadin. This position is responsible for keeping the INRs of all patients within their appropriate range, using the INR ranges and protocols set by the physicians. The LPN collects INR test results for each patient using local lab results, home monitor results and in office INR checks, then documents INRs, dosing instructions, and patient feedback into the patient chart. This position will also include additional nursing responsibilities as needed.


Duties and Responsibilities

  • Demonstrates exceptional care and compassion in all interactions with patients.
  • Ensures all information and proper documentation provided to physician via EMR and records are accurate and up to date.
  • Patient education based on current lab results; every patient is to be called with normal and abnormal findings, contacting non-compliant patients and documenting in chat when patient is called. Mail letters following protocol.
  • Interacts with outside medical offices, diagnostic centers and other
    companies/organizations based on the needs of the patient.
  • Handles all Warfarin related Triage calls and relays information to
    patient per recommendations and direction of provider.
  • Handles all billing associated with the Coumadin clinic.
  • Assists in refilling all Warfarin and Lovenox prescriptions.
  • Assists with cardiac clearances by giving Warfarin hold and Lovenox
    dosing instructions.
  • Assists patients with obtaining INR home monitoring devices.
  • Assists with patient transition to and from Warfarin to other
    anticoagulation medications.
  • Additional nursing duties may be assigned as necessary outside of Coumadin responsibilities.
  • Perform assigned work safely; report to manager in a timely manner
    any unsafe activity, conditions, hazards or safety concerns that may
    cause injury to oneself, other employees, patients or visitors.

 

 Qualifications and Educational Requirements

  • High school diploma or equivalent.
  • Graduate of recognized Medical Assistant Program or holds Medical Assistant Certification.
  • 2 years of experience as an LPN is preferred.

Knowledge, Skills, and Abilities

  • Knowledge of medical terminology.
  • Knowledge of basic math, in particular working with percentages.
  • Ability to multi-task and be adaptable.
  • Excellent communication and customer service skills.
  • Excellent verbal and written communication skills.
  • Ability to prioritize and have great attention to detail.
  • Ability to function in a high paced and at times stressful environment.

 

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
  • Finger dexterity to perform duties such as typing or writing.
  • Ability to speak and hear.
  • Must be able to lift up to 15 pounds at times.
