Job description

The Adult Day Care Center located at Waters Place, Bronx is currently seeking a part time Registered Nurse ( RN)  to join our team! The work schedule is 30 hours per week, Monday to Friday  from 9 am to 3:30 pm, with an occasional Saturday.  The part time RN is entitled to  receive a sign-on bonus of $2500.

Job Summary: Responsible for providing and coordinating multidisciplinary health services for older adults in the Day Care Center . Works with the physician and other team members in implementation of plan of care.

Duties and Responsibilities:

  • Conducts  comprehensive clinical assessments  identifying the service needs of each registrant upon intake and thereafter on a quarterly basis
  • Ensures that for each registrant the findings of the Nursing evaluations, the Nursing Care plan and the recommendations for Nursing follow-up are documented in the clinical record
  • Ensures that MD orders are completed in accordance within the specified time frame; follows up regularly to ensure receipt of MD orders
  • Provides medication administration, prefills, and other skilled Nursing services in accordance with MD orders
  • Assigns CAN tasks in accordance with the Care Plan and oversees the daily completion of the tasks
  • Provides and coordinates cost-efficient patient care that is under the patients’ insurance plan and approved by a physician.
  • Uses diagnosis coding skills to accurately rate the acuity of each registrant
  • Evaluates patient diagnosis and works with Clinical Supervisor, Patient Services, Medical Director,  and physician to ensure clinical services and Nursing interventions meet the goals of the comprehensive Care plan
  • Participates meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team. 
  • Communicates frequently with the clinical team to ensure adherence to services at the frequency and duration ordered by the physician and outlined in the comprehensive Care Plan.
  • Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family.
  • Documents content and process of all services provided.
  • Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs.
  • Appraises care provided, revises goals and interventions to patients’ care as needed.
  • Educates the older adults and family regarding the disease process, self-care techniques and prevention strategies.
  • Initiates and maintains written and verbal communications to assure optimal patient care.
  • Performs other relevant duties as needed.
