Comprehensive Community Services (CCS) Program
Location: Dane County, WI
Schedule: Full-Time, 40 Hours per Week

Make a Difference in Mental Health Care
Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a compassionate and dedicated Service Facilitator to join our Comprehensive Community Services (CCS) team in Dane County. This community-based role supports children, adolescents, adults, and families experiencing mental health challenges through a person-centered, recovery-focused approach.
As a Service Facilitator, you will coordinate services, provide skill development and psychoeducation, facilitate treatment planning, and help individuals achieve greater independence and well-being in their homes, schools, and communities.

Compensation
Compensation is based on education and credentials:
  • Master's Degree: $27.20/hour
  • Master's Degree with In-Training Therapy License: $28.20/hour
  • Master's Degree with Full Clinical Licensure: $30.20/hour

What You'll Do
Care Coordination & Service Facilitation
  • Conduct functional assessments and re-assessments.
  • Develop individualized service plans in partnership with clients, families, support systems, and providers.
  • Coordinate and authorize services as appropriate.
  • Facilitate recovery-focused and family-centered team meetings.
  • Connect clients to community resources and support services.

Documentation & Collaboration
  • Maintain accurate client records and documentation.
  • Complete required reports and service documentation on time.
  • Collaborate with treatment teams, community providers, schools, healthcare professionals, and natural supports.
  • Participate in supervision, training, and staff development opportunities.

What We're Looking For
Required Qualifications
  • Master's degree in a human services-related field.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout Dane County to meet clients in community settings.
  • Ability to work flexible hours, including occasional evenings based on client needs.

Preferred Qualifications
  • Experience providing mental health, behavioral health, therapy, case management, care coordination, or community-based services.
  • Experience working with children, families, and adults with complex needs.
  • Experience in recovery-oriented, trauma-informed, or person-centered service delivery.
