Job description

Overview

The QIDP works within an ICF (Intermediate Care Facility) to ensure each client's active treatment program is integrated, coordinated, and monitored. The QIDP works directly with individuals and paraprofessionals, nonprofessional and other program staff who work with individuals. They must be able to participate as part of the interdisciplinary team in relevant aspects of the active treatment process. Also participates in on-going staff development and training in both formal and informal settings. They will ensure the individuals receive the highest standard of services required. They will ensure that all services are coordinated and implemented as needed and must be able to travel, have good writing skills, articulate and be an aggressive advocate. Responsibilities
  • Coordinate and conduct Annual and Semi-Annual reviews of individual status/progress including an assessment of risks to individual's rights and protection.
  • Coordinate and direct the delivery of training, the provision of medical/nursing and habilitation services. Monitors the therapeutic needs and ensures that all areas are in compliance with statutory and regulatory requirements.
  • Ensure completion and receipt of individual clinical team members' reports/meetings evaluations.
  • Prepare annual and semi-annual CFA's, Rehab plans and summaries which include a capability needs assessment, priority of needs and a summary of clinical reports, activities schedules and individual profiles.
  • Ensure the least restrictive interventions or practices are utilized.
  • Ensure individuals identified as having practices, interventions and or adaptive equipment, which may pose a risk to individual's rights and or protection, are presented to the Human Rights Committee for their review and or approval.
  • Work with the interdisciplinary team regarding changes in individuals' diets and nutritional foals and supply I.D.T. staff with nutritional information and diets plans for residents going on home visits or being discharged.
  • Complete observation of the Direct Support Professional conduct goals active treatment with and provide hands-on training as needed or prescribed consumers by facilities Plan Of Corrective Action (POCA)
  • Provide staff with ongoing in-service training.
  • Ensure integration of services between Day Program and the residence and, integration of all services into routine daily activities.
  • Complete CSET schedule annually and update as needed. Also ensure CSET training is provided and reported on by clinicians as needed.
  • Coordinator and conduct weekly meetings with the clinical team.
  • Initiate periodic reviews as per regulations and conduct all quarterly, annual, and special individual case conferences.
  • Responsible for conducting investigations, providing documentation and timely reporting on any and all incidents, as per OPWDD regulations. 
  • Complete Quality Improvement reports as needed and ensure resolution to any outstanding issues.
  • Maintain communication with family/correspondent in regards to residential programming in conjunction with Program Coordinator.
  • Function as a liaison between all aspects of the resident's program, in-house, day program, family and community services ensure coordination and integration.
  • Collect all written materials from other professionals responsible for aspects of the individual's plan of care and assemble them into the required regularly occurring service plans.
  • Integrate various aspects of the facility's programs.
  • Initiate periodic reviews as per regulations.
  • Oversee the development and implementation of the individual's program, including all behavioral goals and objectives as well as agency actions and staff services.
  • Collect all written materials from other professionals responsible for aspects of the individual's plan of care and assemble them into the required regularly occurring service plans.
  • Responsible for assuring that monthly progress notes are comprehensive and complete, Q.I.D.P. member/staff observations, demonstration of behavior management problems and "hands on" training of staff.
  • Be available to assist with Behavior/Psychological problems.
  • May advise physicians in the availability of diet plans used at the facility.
  • Participate and cooperate with day program component by providing information sharing, consultation, demonstration, and training.
  • Hours are flexible; however a weekly schedule is required and QIPD must be present at all IDT meetings.
  • Additional responsibilities as deemed necessary and appropriate by the Director of DDS and /or Designee. 
Qualifications
  • BA Degree in Psychology, Human Services, or related field.
  • 2-5 years experience within a Day Hab or Residential program.
  • Must have experience with Day Habilitation plans and Medicaid billing.
  • Must be detail oriented and have exceptional written/oral communication skills.
  • Valid Driver's license preferred.
  • SCIP-R and First Aid/CPR Certified
