●        About Facility:

o   This is a hospital affiliated outpatient therapy clinic specializing in occupational, physical, and speech therapy. At this location we have teams of therapists specializing in Orthopedic, Neurological, and Sports therapies.

●        Department Information:

o   Census? Average patients/day? 10-15 patients per day. This clinic schedules 30 min intervals and dovetails patients. Complex evaluations will be 1-hour, simple evals and treatments will be 30 min.

o   Minimum Years of Experience required? Prefer 1 year of relevant experience

o   Will you accept a first-time traveler? Yes

●        Patient Types/Common Diagnoses:

o   General orthopedics, debility, stroke/CVA, neurological disorders, multi-trauma, fractures, Sports injuries

●        Required License, National Certification, Certifications (BLS, ACLS, NIHSS, etc.), or other position specific requirements such as Driver's License and/or Insurance:

o   Licensed in the state of Georgia, BLS

●        Skills required:

o   This location has 27 therapists with various certifications (OCS, SCS, CHT, Pelvic). This clinic also has residents for our OCS program. Therapy Manager, Office Coordinators, Office Manager, and Rehab Aides on site.

o   EMR: EPIC

o   Department specific Equipment: BITS, Solo Step, Force Decks, Cybex Equipment, etc.

●        Floating: If yes, where?

o   No

●        Orientation (facility and unit):

o   NEO

o   5 days departmental

●        Scheduling:

o   Weekend rotation: No

o   On call? If so, what is that schedule? No

o   Holiday Expectations: None

o   RTO Requests: If participating in SimpliFi Clinical Interviewing – can we approve up to 7 days of RTO on your behalf, if traveler extends contract to accommodate? Yes

o   Shift times: 5-8’s or 4-10’s (clinic may determine day off that will be consistent throughout assignment)
