Position Summary

This is a work from home flexible position with 85% travel requirements around Burlington, Ocean, and Monmouth Counties, New Jersey (50-mile radius from applicants' home).

· Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.

· Help members access vital services such as:

o Adult Medical Daycare

o Personal Care Assistance

o Nursing Facility Custodial Care

o Personal Preference Program

o MLTSS Program Enrollment

· Complete 10+ assessments per week (based on need) using a holistic approach that considers co-morbidities, multiple diagnoses, and functional limitations.

· Collaborate with your team and supervisor to overcome barriers and achieve goals.

· Use your interviewing skills to engage members and uncover their health needs.

· Mentor new team members once you've mastered your role.

· Operate at full capacity by month four with comprehensive training and support.

· Assist with developing a proactive plan of care to address identified issues to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness.

· - Uses a holistic approach to assess the need for a referral to clinical resources and other interdisciplinary team members.

Required Qualifications

· Must reside in New Jersey. Case Load and Travel will be spread across Burlington, Ocean, Monmouth, and surrounding Counties

· Must possess reliable transportation and be willing and able to travel up to 85% of the time. Mileage is reimbursed per our company expense reimbursement policy

· Active, unrestricted RN license, New Jersey.

· 1 or more years clinical experience (hospital, home health, or ambulatory care).

· Comfortable working with diverse populations in various settings.

· Strong attention to detail, organizational skills, and ability to work independently.

· Computer proficiency and experience handling medical records.

· Ability to work Monday-Friday, 8:00 AM-5:00 PM EST. No weekends or Holidays.

Preferred Qualifications

· Assessment, case management, or discharge planning experience.

· Managed care or crisis intervention background.

· Strong interviewing and communication skills.

· Proficiency with MS Office Suite and EMR systems.

· Successful completion of the NJ Choice Certification, including Minimum exam score of 80%, Completion of state training modules and Field mentoring component

  • NJ Choice Certification training will be provided if needed

Education

Associate Degree in Nursing required; BSN preferred.

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
