Position Purpose:

The [Nurse Senior - Case Mgmt] works in a collaborative environment at Holy Cross Health within the [Unit/Dept] to deliver excellent patient care as part of an interdisciplinary team providing evidence-based medicine and individualized patient care. This position is a great opportunity to work in an organization that focuses on treating the whole person physically, emotionally and spiritually.

Must be authorized to work in the United States without work visa or sponsorship.

What You Will Do:

  • Employment Type/Shift: FT/Days

  • Using Quality Caring Model, demonstrates acceptable performance, and applies nursing process, to plan and manage care of routine patients, including patient and family education, and continuum of care planning.

  • Implements case management activities for all age groups that promote continuous improvement in the coordination of care (birth to geriatric).

  • Monitors and manages clinical resources ensuring clinical documentation is accurate.

  • Provides patient and family education about disease management and expected outcomes.

Minimum Qualifications:

  • Graduate from an accredited nursing program ADN; basic professional nursing degree (BSN or MSN) preferred.

  • Current Registered Nurse licensure by the Maryland State Board of Nursing or Compact State.

  • Certifications aligned with area of responsibility preferred.

  • Minimum 4 years of RN experience preferred.

FT/PT Benefit eligible Roles:

0.5 FTE (20 hours weekly) up to 1.0 FTE (40 hours weekly)

Pay Range: $41.46 - $64.31

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
