Client location and schedule hours:
Client located in the Green Hills are. Schedule 7p to 7a M-Su
Job Responsibilities:
 Client  enjoys watching the news on TV but is Very hard of hearing and must use her headphones. She enjoys playing cards and sometimes doing puzzles.  
Diagnoses:  Alzheimer's, Sundowner's, Chronic UTI's, Incontinence issues, Fall risk (please shadow on all transfers). Bilateral Hearing Loss with aids, Edema in lower legs.  Allergies:  None know
Goals & Objectives:  In AM-allow client to wake up on her own. Check her gown and bed for cleanless. Assist out of bed w/gait belt and into bathroom onto toilet.  Offer coffee w/creamer. Assist to brush her teeth, put on her zipper knit suit, socks/slippers/shoes.  prepare and serve all meals, feeding her the first few bites to get her started.  Keep running grocery list & check fridge for expired foods. Keep hydrated by taking water to her and telling her to take a sip frequently. Assist her to find favorite TV shows. When sitting watching TV, help client keep legs elevated by lifting at her heels (not calves). Engage with her as she is comfortable. Light housekeeping. Assist client  with exercises as recommended. May accompany to Dr appts or errands.
ADL's:  Mobility:   Client  needs a lot of help getting up on to her walker and needs to be assisted with a gait belt.  She has gotten weaker in the last few days but make sure to do exercises as possible. Her legs need to be propped up at all times she is sitting due to her lower leg swelling. CG should assist her slowly and needs to stay very close through entire transfer. At times when she is weaker, she can be pushed in her transport chair to the restroom.  
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language 
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
