Key Responsibilities
  • Conduct comprehensive psychosocial assessments for individuals, adolescents, families, and groups, adjusting interventions according to client needs and departmental protocols.
  • Collaborate with the treatment team to develop, communicate, and implement interdisciplinary care plans, ensuring all documentation and communication are legally compliant and ethically executed.
  • Provide effective case management for assigned patients, including consultation and coordination with community professionals and programs to optimize patient care.
  • Deliver therapeutic interventions, including but not limited to Cognitive Behavioral Therapy (CBT), and utilize evidence-based methodologies suited to individual service plans.
  • Maintain timely and accurate documentation of patient progress and treatment outcomes to track effectiveness and inform ongoing treatment strategies.
