Certified Occupational Therapy Assistant (COTA) - Home Health
  • Schedule: Full-time Monday-Friday
  • Location: Field-based- Katy, TX and surrounding areas/Pod
  • Comp: $58 Per Point
Who We Are
Who we are:
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health mode. Our ultimate mission is to dramatically improve patient access to home health care.
The COTA role is the front line of that mission: you deliver high-quality functional visits in the home under the OT plan of care, helping patients stay safe and independent.
We build technology designed around the way clinicians actually work, so you can spend less time on documentation and coordination, and more time with patients.
  • Smarter charting: Record or dictate visits with AI Scribe, start with prefilled assessments, extract medication lists from photos, and keep all visit information in one easy-to-use place.
  • Scheduling that protects your time: Text patients, plan more efficient routes, and stay aligned on visit changes through one clear, coordinated schedule.

The result: less administrative burden, fewer coordination headaches, and more time for patient care.
What you will do:
  • Deliver skilled COTA visits in the home under the supervising OT's plan of care.
  • Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
  • Track objective functional progress and communicate updates/concerns to the supervising OT.
  • Coordinate with nursing and the broader care team: escalate changes in condition and safety concerns.
  • Document promptly and thoroughly.
Why Adaptive
  • Operations that back you up: You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.
  • Clear productivity model with upside: You are paid per productivity point with clear expectations (market-specific).
  • Schedule flexibility: We build schedules to make field work sustainable, and we support smart routing and coordination so your day is predictable whenever possible.
  • Cutting-edge tech built for clinicians: Our charting tools are designed to reduce documentation burden, and our scheduling product (launching soon) will streamline visit coordination and reduce last-minute changes.
  • Mission-driven work: We are here to expand access to high-quality home health care, and we want therapists who take pride in raising the standard.
Requirements
Must-haves
  • Active Texas COTA license (in good standing)
  • Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision)
  • Strong communication and documentation
  • Current driver's license and comfort with daily travel across assigned territory

Nice-to-haves
  • Prior home health experience
  • EMR Experience
