Job Description
Title: Registered Nurse - Home Health
Schedule: Shift: M-F until shift is complete
Assignment Details: Contract Weeks: 9
Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- CHPN certification (Preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
Roles & Responsibilities:
- Assess home health and hospice patients to identify their physical, psychosocial, and environmental needs.
- Develop, implement, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families on care, diet, medications, and home safety.
- Monitor patient progress and ensure compliance with third-party payer requirements.
- Provide effective communication to team members and other health care professionals.
- Supervise and provide clinical direction to home health aides to ensure quality care.
