Responsibilitiesof theCommunity Outreach Specialist

  • Facilitatesand securesenrollments from members in the community toparticipateinWayspring'sservices
  • Acts quickly to assess member needs and triage accordingly
  • Successfully engages members and provides them with an educational overview of Wayspring'sservice offerings
  • Securesverbal and written consent, and obtainsappropriate paperworkfor services,includingconsent to clinic services and release of information forms,from members toparticipateinWayspring's services
  • Secures members' first appointment with clinical services
  • Providesfrequent touchpoints to enrolled caseload and ability to assessindividual member needs
  • Initiatescreative strategies tofacilitate member contact
  • Incorporatesrecovery support and harm reduction themes into engagement
  • Regularly reviewsmember profile forSDoH and clinical updates
  • Maintainsperformance accountability around member enrollmentandclinic conversion, and achievesperformance targets
  • Adheres to Wayspring information security and privacy requirement

Requirements & Preferred Qualifications

  • Minimum of three (3) years of experience in a community outreach role, with preferredexperience around street outreach
    • Candidates must meet one of the following requirements:

      • Possess a bachelor's degree from an accredited college or university; or

      • Be eligible to obtain certification as a Certified Peer Support Specialist in accordance with applicable state requirements.

        • For candidates pursuing Peer Support Specialist certification, eligibility requires self-identification as an individual in recovery from a substance use disorder and the ability to attest to being sufficiently stable in their recovery to provide peer support services. Consistent with industry and auditing standards, candidates are generally expected to have maintained at least one year of sustained recovery prior to providing peer support services.

  • Demonstrated ability working in a matrixed environment
  • The ability to communicate effectively and persuasively is required
  • Strong verbal, written, and presentation skills, with the ability to communicate complex information clearly and professionally
  • Exceptional verbal de-escalation skills and crisis management
  • Experience with CRM, EHR, or similar platforms is preferred
  • Ability to travel as business needs require (community-based role). Mileage reimbursement is provided.
