Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities to identify their specific needs and establish tailored treatment plans.
• Deliver skilled therapy services including therapeutic exercises, manual therapy techniques, and various modalities designed to address geriatric-specific challenges.
• Regularly monitor and document patient progress through detailed charting, making adjustments to treatment plans as necessary to ensure the best possible outcomes.
• Collaborate with other healthcare professionals to create a holistic approach to patient care, sharing insights and strategies to optimize recovery.
• Educate patients and their families on effective therapeutic techniques, home exercise regimens, and injury prevention strategies to promote lifelong health.
• Implement fall prevention programs and other safety measures to minimize risks within assisted living and rehabilitation settings.
• Engage in continuous learning by staying current with advancements in physical therapy and participating in relevant continuing education opportunities.
• Uphold ethical and professional standards, maintaining confidentiality and commitment to the highest level of patient care.

Patient Population / Client Focus

This role is primarily focused on our geriatric clientele, who may present with conditions such as arthritis, balance disorders, post-surgical rehabilitation needs, and chronic pain. Understanding the delicate nature of this demographic is crucial as you tailor your interventions to promote their comfort, independence, and overall quality of life. Your work will significantly contribute to their ability to age in place successfully.

Qualifications

• Must hold a Bachelor's degree in Physical Therapy from an accredited university.
• Possess an active state licensure as a Physical Therapist.
• Strong interpersonal and communication skills to effectively engage with patients, families, and multidisciplinary healthcare teams are essential.
• Proficient in electronic medical records (EMRs) to maintain documentation integrity and facilitate efficient workflow.
• Commitment to evidence-based practices, with the ability to incorporate the latest therapeutic methods into your approach.

Preferred Experience

• At least one year of experience in geriatric therapy is preferred; however, new graduates with a passion for this area are encouraged to apply.

Compensation and Benefits
