The Hospice Weekend Registered Nurse (RN) Coordinator provides compassionate, patient-centered care to hospice patients and their families during weekend business hours. This role is responsible for performing scheduled patient visits, completing hospice admissions, and conducting consults to evaluate eligibility for hospice services. The RN collaborates with the interdisciplinary team to ensure quality care, symptom management, and emotional support for patients facing life-limiting illnesses.
Key Responsibilities:
  • Perform scheduled hospice visits to assess patient status, provide nursing care, and manage symptoms.
  • Complete hospice admissions, including comprehensive patient assessments, documentation, and initiation of the hospice care plan.
  • Conduct hospice consults to evaluate patient eligibility and educate patients and families about hospice services.
  • Provide pain and symptom management in accordance with hospice philosophy and physician orders.
  • Educate patients, caregivers, and families about disease progression, medications, comfort measures, and end-of-life care.
  • Document patient assessments, interventions, and outcomes in the electronic medical record in a timely manner.
  • Respond to changes in patient condition and communicate updates to the hospice care team.
  • Ensure compliance with Medicare hospice regulations, agency policies, and standards of practice.
  • Provide emotional support and guidance to patients and families.
  • Conduct in-facility assessments in accordance with Suburban Hospice admission policies and procedures; may evaluate and admit patients to inpatient hospice care.
  • Collaborate with interdisciplinary group (IDG) team members to ensure high-quality patient care and patient/family satisfaction.
  • Complete documentation in the electronic health record (EHR) in accordance with Suburban's clinical policies and procedures.
  • Evaluate patient and family needs and assist in preparing and implementing the individualized plan of care for each patient.
  • Communicate identified needs and potential solutions to the Administrator On-Call.
  • Perform other duties as assigned.

Requirements
Basic Qualifications
  • Must be a Registered Nurse (RN) licensed by the Indiana Professional Licensing Board and a graduate of an approved nursing program.
  • Must have at least 2 years of experience as a Registered Nurse and at least 1 year of hospice experience.
  • Must possess a valid driver's license and automobile insurance.
