About the Practice
Breast Surgery Group is a boutique plastic surgery practice in Midtown Manhattan specializing in breast reductions, breast lifts, and top surgery (chest masculinization and breast augmentation for transgender and non-binary patients). Our clinical team includes two board-certified plastic surgeons and two physician assistants. We are in-network with Cigna and accept out-of-network benefits for most other plans.
Position Overview
We are seeking an experienced Medical Biller to join our revenue cycle team. This role owns the full billing lifecycle — from claim submission and payer follow-up through payment reconciliation and AR management — with particular emphasis on out-of-network billing, No Surprises Act (NSA) compliance, and the Independent Dispute Resolution (IDR) process. The position works closely with our external RCM partner and reports to the Practice Manager.
Key Responsibilities
• Submit clean claims to commercial payers for surgical and office-based services, including both in-network (Cigna) and out-of-network cases
• Manage the full NSA billing workflow: good faith estimates, Explanation of Payment (EOP) review, and required notices to patients
• Submit NSA claims to CMS and manage the federal and New York DFS IDR processes from initiation through resolution
• Track and reconcile IDR case statuses; coordinate with our RCM partner (CH Revenue Management) on arbitration strategy and invoice documentation
• Conduct payer follow-up on unpaid and denied claims; write and submit appeals with supporting clinical documentation
• Monitor AR aging; identify trends in denials, underpayments, and NSA cases and escalate appropriately
• Verify insurance eligibility and benefits, including out-of-network benefit parameters, prior to scheduled procedures
• Post payments, contractual adjustments, and patient responsibility balances accurately
• Collaborate with PAs and clinical staff to obtain documentation needed for prior authorizations and appeals
• Maintain compliance with HIPAA, payer contracts, and applicable state and federal billing regulations
• Support monthly reporting on collections, denial rates, IDR outcomes, and payer mix for Practice Manager review
• Address all mail pertaining to billing
• Manage payor takebacks
Required Qualifications
• Minimum 2 years of medical billing experience in a clinical or surgical setting
• Demonstrated experience submitting NSA claims to CMS and managing the IDR process (federal and/or NY DFS)
• Working knowledge of out-of-network billing, Qualified Payment Amounts (QPAs), and FAIR Health benchmarks
• Proficiency with CPT and ICD-10 coding for surgical and office-based services
• Experience with AR management, denial management, and payer appeals
• Familiarity with commercial insurance EOBs, coordination of benefits, and patient responsibility calculations
• Strong attention to detail and ability to manage a high-volume, multi-payer claims environment
• Effective written and verbal communication skills; ability to work independently and meet deadlines
Preferred Qualifications
• Plastic surgery or surgical subspecialty billing experience
• Certified Professional Coder (CPC), Certified Coding Specialist (CCS), or equivalent coding certification
• Experience working with an external RCM partner or billing service
• Familiarity with New York DFS IDR process in addition to federal IDR
Work Arrangement
This is a full-time, in-office position based at our Midtown Manhattan location. Candidates must be able to reliably commute to the office five days per week.
Compensation
Salary range: $60,000 – $80,000 annually, commensurate with experience. Benefits package available.
