Job description

SpringPath Mental Health Services, a part of the Sevita family, provides behavioral health services through Intensive Residential Treatment, Assertive Community Treatment, and Targeted Case Management programs. We embrace a person-centered approach that is trauma informed, culturally responsive, and recovery-oriented. Our dedicated team supports individuals with mental health, psychiatric, or other complex needs to overcome challenges, build skills, and live a full, meaningful life.


SUMMARY

The Psychiatric Rehabilitation Practitioner provides assistance to individuals with mental health conditions in their journey towards recovery and independence. They work closely with clients to develop personalized rehabilitation plans, focusing on areas such as daily living skills, employment, education, and socialization. The practitioner utilizes various therapeutic techniques and interventions to help clients manage their symptoms, develop coping strategies, and improve their overall well-being.

ESSENTIAL JOB FUNCTIONS

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.  

  • Conduct a variety of assessments including crisis, medical/behavioral emergency (such as suicidal), veteran affairs, immediate needs, functional, living skills, and resource acquisition.
  • Complete interpretative summaries and develop crisis/treatment/behavioral plans and goals for clients.
  • Write progress notes, summaries, and discharges.
  • Lead group and individual counseling services utilizing skills teaching, case management, and household management.
  • Implement the treatment/rehabilitative activities and strategies including crisis prevention and medication administration.
  • Participate in weekly staff meetings.
  • Report significant / major incidents or accidents.
  • Establish high levels of mutual trust and support with others. Provide coaching/learning environment.
  • Provide and promote consumer choice and facilitates independence.
  • Promote the operation in a positive manner to the community and outside agencies.
  • Promote/encourage community integration and choice making.
  • Notify proper authorities of circumstances or situations that present possible health hazard.
  • Observe and implement established policies on health, safety, security, code of conduct, regulation, and company policies.

Minimum Knowledge and Skills required for the Job

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.

 

Education and Experience: 

  • Bachelor’s degree in one of the behavioral sciences from an accredited college or university.
  • Minimum of 2000 hours (post Bachelor degree) of verifiable work experience providing services and support to individuals with mental illness.
  • Master’s degree in one of the behavioral sciences from an accredited college or university preferred.
  • Supervisory experience preferred.
  • Experience in managing systems, processes, and people.

 

Certificates, Licenses, and Registrations:

  • Valid driver’s license and car insurance required.

 

Other Skills and Abilities:

  • Able to perform duties independently and as a team.
  • Ability to communicate verbally and in writing.
  • Operate general office equipment skills.

 

Other Requirements:

  • Travel as needed.

 

Physical Requirements:

  • Sedentary work: Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.  Sedentary work involves sitting most of the time.  Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Salary: $55,000-$58,000
