Job description


Our client is looking for a Peer Specialist/Community Health Worker to support OUD/SUD members by addressing medical, behavioral, and social needs to improve overall well-being. The role involves connecting members to physical and behavioral health services and linking them to essential community resources.
Responsibilities:
  • Engage with members in the community (60-80%) and by phone (20-40%), providing support, encouragement, and positive reinforcement.
  • Conduct comprehensive assessments covering clinical, psychosocial, and behavioral health needs.
  • Use motivational interviewing and active listening to help members identify goals and develop personalized care plans.
  • Assist members with scheduling appointments, filling prescriptions, and accessing behavioral health services.
  • Connect members to resources addressing social determinants of health and help resolve barriers to care.
  • Collaborate with providers and MCOs, presenting complex cases and obtaining input for member care plans.
  • Document interactions, assessments, and referrals in relevant systems and stay updated on technology changes.
  • Follow up to ensure members understand their medication and care plans.
  • Meet performance goals within community and virtual environments.

Requirements
  • Peer Specialist Certification, lived experience with SUD/OUD/Mental Illness, or 1-2 years of experience as a Peer Support Specialist.
  • High school diploma or GED.
  • Passion for supporting individuals with complex medical, behavioral, or social needs.
  • Strong knowledge of SUD/OUD member management and experience in community or clinical settings.
  • Excellent communication and organizational skills, with the ability to multi-task in a fast-paced environment.
  • Proficiency in Microsoft Word and Outlook; ability to learn clinical applications quickly.
  • Access to a designated workspace with secure high-speed internet.
  • Valid driver's license and auto insurance; ability to travel for face-to-face visits and navigate Philadelphia's public transit.
