The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Develops, implements and oversees the assigned program(s). Responsibilities include development and care coordination for patients, including ongoing education to the patient/family, care teams and/or community resources, regular auditing to identify and resolve clinical issues which affect patient care, clinical efficiency, and effectiveness, and making programmatic improvements through collaboration, project management and performance improvement activities. May supervise 1-2 staff.
ESSENTIAL FUNCTIONS
Staff Leadership:
  • Assists with QA/PI initiatives by acting as a liaison to the patient care team.
  • Fosters effective communication with employees, peers, and leaders; builds and maintains relationships; develops others; and manages projects, team and work groups.
  • Assists daily workflow of QA RNs; ensuring timely and accurate data collection, submission and reporting to requisite regulatory divisions, submission of case reviews, reporting QA to the Department of Surgery.
  • Monitors staff engagement and initiates actions to ensure high levels of engagement. Articulates goals and coaches staff for optimal performance toward stated goals.
  • Communicates quality standards/initiatives to QA and clinical teams and other appropriate staff.
  • Assists NM with processes for data validation, ensuring data integrity and maintaining confidentiality of registry data.
  • Models Strong Commitment behaviors and encourages staff to follow by example.
  • Provides QA/PI coverage in lieu of NM when indicated

Program Leadership:
  • Ensures multidisciplinary quality care and performance improvement activities within the division are in accordance with established reporting requirements.
  • Makes significant contributions to Compliance Plans, and drives quality initiatives and performance improvement throughout the organization.
  • Participates in root cause analysis for NYS Reportable Events, near miss events.
  • Assists with Division compliance with hospital QA policies/guidelines, and other standards.
  • Coordinates and leads conversations with patient relations, office of counsel to address/resolve patient complaints/grievances
  • Participates in standardization of practice guidelines making recommendations for improvement
  • Provides direct educational programs for clinical staff.
  • Serves as an internal quality resource for staff, in all departments.
  • Compiles audit summaries at regular intervals with dissemination to appropriate care team(s), including feedback regarding successful implementation and areas of opportunity.

Registry Management
  • Accurately identify surgical patients for inclusion in the database through the application of strict inclusion/exclusion criteria and protocols
  • Performs meticulous chart review and enters clinical data into the quality database to identify opportunities for improved outcomes, reduced cost, improved LOS and best practice utilization of resources.
  • Ensures complete and accurate collection of preoperative, operative and postoperative data through the effective utilization of the EMR and other Hospital databases.
  • Ensures appropriate utilization of resources necessary for accurate and timely data entry into the program's database.
  • Demonstrates applicability of the methodology and the reliability of definitions utilized by reviewers within the program through inter-rater reliability, (IIR) testing
  • Serves as an educational resource on the Vascular Quality Program.
  • Identifies areas for streamlining and process improvement in the data collection process.
  • Prepares reports utilizing the database as requested.
  • Attends Department and Division level Quality Meetings/Conferences.
  • Participates in Regional Collaborative biannual meetings.
  • Participates in scheduled teleconferences with other surgical clinical nurse reviewers.

Project Nurse/Lead Quality Analyst
  • Utilizes data sources to analyze patient outcomes, identify patterns and trends in care delivery in order to create safety/quality initiatives for hospital patients.
  • Develops project work-plans using Quality Improvement principles.
  • Utilizes evidence-based practice to facilitate ongoing quality improvement efforts to drive better patient outcomes.
  • Participates in multidisciplinary teams on quality improvement initiatives.
  • Regularly reports quality improvement activities and quality outcomes to providers and clinical staff and is responsible for presentation of this work at various Division, Department, and Hospital level committees.
  • Participates in root cause analyses and creates required documents as needed.
  • Completes audits on patient records and clinical documentation for quality purposes.
  • Assists with work related to the Quality Collaboratives the Department is involved with.
  • Maintains knowledge of current trends, issues and research in Quality Improvement.

MINIMUM EDUCATION & EXPERIENCE
  • Bachelor's degree in Nursing and 5 years of professional nursing experience with progressive clinical experience required
  • Or equivalent combination of education and experience
  • Experience working as a member of a multi-disciplinary team required

KNOWLEDGE, SKILLS AND ABILITIES
  • Ability to facilitate and lead a team approach to population based care preferred
  • Excellent interpersonal and communication skills, including written communication preferred
  • Strong interpersonal, delegation and problem-solving skills preferred
  • Strong analytical and data management skills preferred
  • Detail oriented with strong organizational and prioritization skills and ability to work independently under general guidance preferred
  • Electronic Medical Record (EMR) skills preferred
  • Basic proficiency with Microsoft Outlook, Word and Excel preferred
  • 3 years of clinical chart review, monitoring clinical data/performance and performance improvement preferred
  • Quality improvement or patient safety experience preferred
  • Supervisory/management experience preferred

LICENSES AND CERTIFICATIONS
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure in New York State upon hire required
