Position Description:
At St. Gabriel's Hospice & Palliative Care, we are looking for compassionate and experienced Registered Nurses to join our team and provide exceptional care to our patients at our Houston location. In this role, you will be responsible for delivering high-quality hospice care, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory requirements.
Responsibilities:
Provide direct patient care in accordance with hospice care plans.
Collaborate with physicians, social workers, and other healthcare professionals.
Conduct patient assessments and create personalized care plans.
Administer medications and treatments as prescribed.
Educate patients and families about care plans and symptom management.
