Roles & Responsibilities
- Utilize the nursing process to assess patient and caregiver needs to provide quality care in accordance with applicable regulations and policies.
- Develop and communicate a realistic plan of care that includes patients and caregivers in the process.
- Serve as Case Manager for assigned patients, actively managing case loads and visit times in line with agency productivity standards.
- Effectively complete OASIS documentation in compliance with agency policies and state and federal regulations, ensuring timeliness and accuracy.
- Implement medical plan of care and nursing interventions to ensure a high quality of patient care.
- Educate patients and caregivers about treatment options, medication side effects, and discharge instructions to support informed decision-making.
