MDS Coordinator – RN/LPN
Near Douglasville, GA

Overview:
Our skilled nursing facility near Douglasville is seeking an MDS Coordinator (RN/LPN) to manage resident assessments, care planning, and regulatory documentation. This role plays a key part in ensuring accurate MDS coordination and survey readiness.

Position Offers:

  • Schedule: Monday–Friday
  • Pay: $68,000–$80,000 per year (DOE)

Key Responsibilities:

  • Complete and coordinate resident MDS assessments according to regulations and facility policies
  • Develop and update individualized care plans based on resident assessments and interdisciplinary input
  • Collaborate with nursing, therapy, social services, and other disciplines to support accurate documentation
  • Monitor assessment schedules and ensure timely completion and submission
  • Review clinical records for accuracy and resolve documentation discrepancies
  • Apply knowledge of Medicare and Medicaid regulations related to MDS and reimbursement
  • Communicate assessment findings and care plan updates with residents, families, staff, and regulatory representatives
  • Support survey readiness by ensuring documentation and MDS records meet regulatory standards

Qualifications:

  • Current RN or LPN license in Georgia
  • ADN required; BSN preferred
  • Minimum 1 year of skilled nursing or long-term care experience
  • MDS assessment, documentation, and coordination experience required
  • MDS/RAI certification or formal MDS training preferred
  • Experience with long-term care EMR systems required
