Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week travel contract in the San Mateo, CA area.

Details
  • Home health setting serving patients across a defined geographic territory including South SF, Pacifica, Millbrae, Hillsborough, Burlingame, San Carlos, Foster City, San Mateo, Redwood City, Emerald Hills, Menlo Park, Mountain View, and neighboring cities
  • Day shift hours from 8amโ€“5:30pm, with schedule flexibility based on patient availability
  • 13-week contract
Requirements
  • Minimum 1 year of home health PT experience required
  • Current and active state PT license required (license required if applicable)
  • BLS/BCLS certification required
Additional Information
  • Responsibilities include evaluating and treating patients using medically prescribed physical therapy programs to restore function and maximize patient performance
  • May supervise Physical Therapy Assistants as part of care team duties
  • Responsible for completing initial admission/evaluation visits and reporting OASIS scores to the Clinical Team Manager within established timelines
  • Documents clinical findings, skilled interventions, goals, and discharge plans in accordance with agency standards
