Job description

Occupational Therapist 

Location: Grand Junction, CO
Pay Rate: $45.00–$50.00/hr
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Contract: 13 weeks
Experience: Minimum 2 years

About the Role

We are hiring an Occupational Therapist for a 13-week contract in Grand Junction, CO. This role supports outpatient and home health services, helping elderly and frail patients improve daily living skills, safety, independence, and quality of life.

Responsibilities

  • Evaluate patients and create individualized occupational therapy treatment plans
  • Provide therapy focused on daily living skills, mobility, safety, and independence
  • Support elderly and frail patient populations in a PACE-related care model
  • Provide services in outpatient and home health settings
  • Educate patients and caregivers on adaptive strategies and safe care techniques
  • Collaborate with interdisciplinary care teams to support patient goals
  • Monitor progress and adjust treatment plans as needed
  • Complete accurate documentation and follow care standards

Requirements

  • Bachelor’s, Master’s, or Doctorate in Occupational Therapy required
  • Active Colorado Occupational Therapist license required
  • NBCOT registration required
  • Minimum 2 years of Occupational Therapist experience required
  • At least 1 year of experience with elderly and frail populations required
  • Outpatient or home health experience preferred
  • Valid driver’s license, reliable transportation, and auto insurance required
  • Ability to work Monday–Friday day shift
